Please print this page and include it with your first shipment after you have called to discuss your restoration needs.

This information is a guideline for customer concerns about shipping, estimating, and liability while your object is in the care of the China and Crystal Clinic, and your signature is necessary.

  1. All work is guaranteed, for one year, for appearance and structure. Said guarantee is void when the property is sold or given to another party.
  2. Items are repaired with durable air-dry finishes and will not discolor as long as they are not subjected to direct sunlight or extreme heat.
  3. Items ARE NOT guaranteed for food related purposes, including eating, drinking, serving and cooking.
  4. WARNING: Insurance is not provided for your item(s). It is your responsibility to insure each item, for its full value while in transit and on our premises. This includes fire, theft, breakage and all other perils.
  5. A 50% deposit is required on restorations over $100 and the balance of restoration fees, plus return shipping, are due before item is shipped and within 30 days of completion of work.
  6. Merchandise not paid for within thirty days after notification are subject to a storage charge of $1.00 per day. After ninety days it will be presumed that you have abandoned the merchandise and it will be disposed of, unless prior written arrangements have been made.

Shipping and Packaging Tips Shipping and Packaging Tips

All items should be sent with insurance offered by the carriers since the China & Crystal Clinic takes no responsibility for damage incurred in transit to us. These packing instructions are no guarantee of safety, though we believe they will minimize damage which can occur.

Please ship all items to: China & Crystal Clinic, 1808 N. Scottsdale Rd., Tempe, Arizona 85281.

  1. We recommend you wrap the pieces yourself for shipping if at all possible, unless you have a reputable packaging service.
  2. Wrap the broken pieces separately in tissue and bubble wrap. Put all small pieces in tissue and put inside a zip lock bag. Make it so the broken pieces will not rub together and cause more damage. You get the idea!
  3. Make sure to put at least 2 inches of bubble wrap around the pieces and fit in a box with styrofoam peanuts.
  4. Double box the item with at least 2 to 3 inches of styrofoam peanuts in-between.
  5. Be sure to include your name, address, phone number and email address (if you have one) inside the box. We will contact you within 24 to 48 hours after we receive the item to give you your estimate of repair.
  6. DO NOT USE NEWSPAPER TO WRAP AND SHIP ITEMS IN!
  7. We do not restore dinnerware for use! We recommend "Replacements L.T.D." for all dinnerware, crystal and silverware replacements. 1 800 (replacements).
